Referral Services

For referral to another medical office or specialist, it is important to make sure your specialist is covered under your plan. Please contact our office for the referral and have the following information ready:

   • Patient's full name and DOB
   • Phone number
   • Name of your current insurance company
   • Name of the specialist
   • Specialist phone and fax number

Referrals may take up to 5 days to process unless special arrangement have been made. Out-of-network referrals may take longer depending on your insurance.

Prior Authorization: Medication & Diagnostic Imaging

Certain prescription drugs and diagnostic imaging require prior authorization (PA) or a medical exception for coverage. If your drug or imaging test requires this step, our office will need to request and receive approval from your insurance company before the drug may be covered under your insurance plan.
